Trump in a lab coat, diagnosing Rosie O'Donnell

The post has no words from Trump. It is a composited image: Rosie O'Donnell in a hospital gown, grimacing with a hand on her forehead, and Trump in a white coat with a stethoscope, holding a clipboard signed "Donald J. Trump, MD." The chart reads "MASSIVE / SEVERE TDS," scores her "98/100 - CRITICAL LEVEL," ticks off "Irrational rage," "Obsessive Trump fixation," "Meltdown triggered by Trump news" and "Complete loss of composure," and is stamped "MASSIVE TDS." What we can analyze is his decision to publish it. It went up at 01:49 UTC, about 9:49 p.m. Eastern the evening before, and it was one of at least five posts that day in the archive with no written text.

What the image does for him

The meme turns a political opponent into a patient and puts Trump in the role of the clinician. This is devaluation carried out through pathologizing: dislike of Trump is recast as a medical disorder, a "syndrome" with a severity score. The image never has to deal with anything O'Donnell has actually said, because it declares her unfit to be taken seriously. It uses the look of clinical authority, a stamp, a numeric score and a symptom checklist, to make the dismissal seem objective. The rhetorical work is ad hominem by diagnosis plus appeal to authority. Trump is both the authority and the man the patient is supposedly obsessed with.

The symptom list reads as projection, and it is the most revealing part of the post. "Obsessive Trump fixation" is charged against a woman Trump has attacked in public, again and again, since 2006. In 2025 he went as far as floating the revocation of her citizenship. Choosing to publish yet another image of her, two decades into the feud and with no recent trigger in the provided events, is itself an instance of the fixation the checklist assigns to her. "Irrational rage" and "complete loss of composure" project onto a caricatured target the traits that critics most often attribute to him. Confidence in the projection reading is medium. A meme like this is standard fare in his supporters' ecosystem, and a volunteer probably made it, so the attribution of motive stops at the act of selecting and reposting it.

Self-casting as physician

"Donald J. Trump, MD" is a small grandiose flourish: a credential he does not hold, worn with a "stern/serious expression." The narrative role is the judge/healer who stands above the disorder he names. He is the sane observer, and she is the case. At eighty, and with questions about his own health and cognition a regular topic of news coverage, casting himself as the one who hands out mental-status verdicts may be a reversal: the scrutiny is aimed outward before it can land on him. That link is inference from context, not from anything in the image, and confidence is low.

Trigger, state and archetype

Nothing in the historian's events for the week involves O'Donnell. The legal week was about the Truth API lawsuit and the Epstein document ruling. The post therefore reads less like a reaction to a new injury than like maintenance: a routine visit to a reliable enemy whose humiliation pleases his audience. The state is grandiose. The image shows no anger from Trump, only cold superiority. The contempt goes to her, and his side of the scene is calm. The sadistic element is mild but real: the joke is her suffering ("in apparent distress"), shown for spectators to enjoy. In archetypal terms this is the Trickster using the forms of institutional medicine as a prop. There is also a King note: the sovereign decides who is sane.

Order and hierarchy

The post defends a hierarchy of legitimacy. Supporters are rational, and critics are sick. By turning "TDS" into a clinical term, it takes one whole category of objection out of political debate: if opposition is a symptom, it needs no answer. That is epistemic closure in its mildest everyday form. No false factual claim is made, but it trains the audience to hear criticism as proof of the critic's disorder.

Violence and threat

The image mocks and threatens nothing. It has no call to action and no dehumanizing language beyond ridicule. Earlier in the feud Trump talked about stripping her citizenship, a state action against a named critic. This post does not revive that, but it keeps her on his list of named targets.

Authorship

With no written text there is no stylometric evidence. The score describes who selected and posted the image, not who made it. A Friday-evening post in Eastern time (he usually spends August at Bedminster), inside a burst of wordless media posts, and aimed at a personal feud that goes back to his pre-political years, all point toward his own choice over a staff communications decision. Taken together they are weak evidence.
